How to apply
Master of Arts and Certificate in Addiction Counseling
Thank you for your interest in applying to Hazelden Graduate School of Addiction Studies.
Some of the application process can be done online and other components will need to be mailed in. All forms are available to you once you start the application process.
Application Process
- You will fill out an initial application.
- You will receive an email from Populibot@populiweb.com with your username. (If you do not receive this email, please check your spam folder.)
- Follow the email link to Populi to set up your password.
- Once you set up your password you will log in to the Populi site and complete the application process.
