Transcript Request
In accordance with the Family Educational Rights & Privacy Act (FERPA) of 1974, your
records cannot be released without your written consent. All outstanding obligations (financial, academic or administrative) due to the Hazelden Graduate School of Addiction Studies (HGSAS) must be cleared before your transcript request can be processed.
Please note:
- We do not accept phone or email requests for transcripts. All requests must be made in writing with the student signature.
- Transcript requests will not be released if there are any financial obligation to the school.
- Each official transcript is $5.00. Payment is required at the time of the request. There is no charge for unofficial transcripts (student copy).
- Payments can be made by credit card, check, or cash. Faxed requests will only be accepted using credit card payment. Requests will not be processed without payment. We accept cash, check or money order (payable to HGSAS), Visa, MasterCard, Discover, and American Express.
- Please allow approximately 3 – 5 working days for processing.
- The Registrar's office does not hold transcript requests until grades are posted. Please submit your transcript request after your grades are posted.
- Transcripts are sent through the U.S. First Class Mail. Each is mailed in a separate sealed Hazelden envelope. Do not open the envelope if you are forwarding to another party (institution, employer, etc.) Open the envelope only if you have ordered for personal use. Note: Multiple transcripts (each in its own sealed Hazelden envelope) sent to a single address will be enclosed in a manila envelope; you may open the manila envelope.
